We will send you a welcome packet containing:
  • a welcome letter
  • a medical history form
  • a patient information form
  • a map to our office

On Arrival:

When you arrive, you will be greeted by Dr. Martin and he will bring you to our exam room.  Dr. Martin will then conduct a thorough examination to help determine your orthodontic needs.  Some of the things that will be evaluated include:
  • primary concerns
  • alignment of your teeth
  • appearance of your smile
  • facial harmony
  • bite and jaw alignment
  • tooth function during chewing
  • gum health
If braces, retainers, or Invisalign appear to be needed, Dr. Martin will discuss the following with you:
  • possible courses of therapy
  • estimated length and cost of treatment
  • any additional questions or concerns you may have
 
If you are interested in orthodontic treatment after the examination, we recommend allowing us to take diagnostic record.  The time will be reserved to take these records at no additional cost to you.  These records will consist of x-rays, photos, and impressions for study models.  They will provide all of the information to develop a treatment plan designed for you. 

Follow Up:

At the second appointment, the patient conference, Dr. Martin will go over your finalized treatment plan with you.  These appointments will take place over lunch hour or at the end of the treatment day.  The goal of this appointment is for you to gain a clear understanding of your specific needs, treatment details, cost of treatment, and how long it should take.
